Home Health Services

The Region provides health care in your home whether you have just returned from a stay in the hospital or are experiencing temporary health problems. While receiving excellent, professional care, you are able to remain in your home with your family and avoid being institutionalized. You may receive one or more of our services, depending on what you and your doctor determine is most suitable for your recovery.

Skilled Nursing

A Public Health Nurse will coordinate and monitor all phases of patient care, including provision of medical services and teaching disease process, prevention, and appropriate intervention to patient and caregivers.

Physical Therapy

A Physical Therapist will help to restore strength, improve motion, and relieve pain of disability.

Occupational Therapy

An Occupational Therapist will teach you ways to make your home more accessible and train in use of equipment for activities of daily living.

Speech Therapy

A Speech Therapist will provide therapy to rehabilitate speech, swallowing, and voice disorders.

Medical Social Workers

A Medical Social Worker will provide assessments, counseling, education, advocacy, group-work, resource linkage, and community organization. Services are geared toward the prevention, early identification, and alleviation of psychosocial issues impacting the use and response to health services.

Public Health Assistants

Public Health Assistants will assist with medical monitoring, personal grooming, hygiene, meal preparation, walking, exercising, and light house keeping in the room where the patient resides.

Other Services

Wound Resource Nurses
Specially trained Wound Resource Nurses are available to care for acute and chronic wounds, ostomies, foot care, and monitor tube feedings.

Diabetes Educators
Certified Diabetes Educators will teach you and your caregiver to take charge and control diabetes and assist with acquisition of supplies.
