Examples of Partnerships

  1. Public Health Liaison: a Public Health Professional is assigned to a medical practice, in some instances the staff is actually stationed in the office to do assessments for Family Support Services (FSS) to identify the need for Family Support and coordinate referrals to other FSS Providers.
  2. FSS Model: FSS provider works with the partnership on a regular basis (weekly visits, regular phone contact to practice, etc.) to receive and coordinate referrals.
  3. Nurse Practitioner Model: a nurse practitioner is assigned to the partnership to do medical management including EPSDT and acute care.
  4. EPSDT (Early and Periodic Screening Diagnosis and Treatment) Model: a health department nurse is assigned to the practice to do EPSDT (well child care as defined by the Department of Health and Human Services and supported by the American Academy of Pediatrics) physicals.
  5. FSS Triage Model: an RN is assigned to provide FSS for children referred by the practice. The RN is on call for seven (7) days a week from 8:00AM – 10:00PM. Referrals to other disciplines are made as indicated, WIC is provided by the RN for patients being followed through FSS.
  6. WIC (Women Infants and Children, Nutritional Program) Model: at the request of the primary care provider, the health department establishes an out-stationed WIC clinic in the physician’s office.
  7. Health Education Class Model: Classes are developed at the request of the practice and offered within the physician’s office for clients who need them. Classes are provided on topics such as asthma management, weight control, diabetes management, healthy lifestyles, child development, and safety to name a few possibilities.
  8. Post Partum Newborn Home Visit Model: Health Department makes visits to all infants admitted to the practice and links families to FSS as indicated.
