Health Systems Interventions

national diabetes education program logoThe SCDPCP Health Systems interventions are focused on Community Health Center's diabetes treatment and control activities across the state. The goals of these interventions are to improve diabetes care in office-based practices in medically undeserved areas of the state, and to increase diabetes self-management skills of patients who attend these primary care centers. Priority populations are minorities, the elderly, the uninsured and the underinsured.

The SCDPCP provides and/or offers up-to-date information on evidence-based diabetes prevention, screening, diagnosis, treatment, and control in ongoing processes by means of training sessions, symposia, and mailings of provider and patient education materials.

The Community Health Centers are using the Chronic Disease Model and a quality improvement model to make changes and improvements in their health system organization, delivery system, decision support, information systems, community resources and patient self-management. Counties served: Darlington, Marlboro, Chesterfield, Dillon, Lake View, McCall and Lee
Clinic sites: Hartsville, Society Hill, Bishopville, and Cheraw
Special services: Diabetes support groups and diabetes self management classes are held at each site. Each site has a Chronic Disease Care Manager on site.

Family Health Centers, Inc.
Counties served: Orangeburg, Calhoun, Upper Dorchester and Bamberg
Clinic sites: Orangeburg, Vance, St. Matthews, Springfield, Norway, St. George and Harleyville
Special services: Diabetes support group meeting held on the 2nd Tuesday of every month at the Orangeburg site. Diabetes self-management classes held at all sites. Sugar Patrol Project: conduct community diabetes screening and organize health fairs throughout the faith community and with the regional office.

Special services: Diabetes Wednesday and diabetes classes at the Charleston site. Diabetes classes on Thursday at the North Charleston Enterprise Site

Special services: Diabetes day on the first and third Wednesday of each month at Sheldon Health Center.
