Permit Preprinted Discharge Monitoring Reports


Facilities are required to monitor their discharges for compliance with their permit's effluent limitations.  The monitoring frequencies and reporting requirements are specified in the facility's NPDES permit. NPDES permits normally  require monthly self monitoring for most pollutants.  Less frequent monitoring, such as quarterly, semi-annually, etc., may also be specified, as appropriate.  The reporting requirements usually mirror the monitoring requirements.  For example, monthly sampling results are submitted to the Bureau of Water on a monthly basis.  Also, self monitoring results must be submitted to the Bureau on the standard Discharge Monitoring Report (DMR) form.  This form contains blanks for filling in the permittee's name, address, NPDES permit number, outfall number, reporting period, pollutants limited in the permit with the corresponding limits, sample type, sample frequency, etc.

To assist permittee's in meeting their reporting requirements, the Bureau provides preprinted DMRs to each permittee.  These preprinted DMRs have all the information filled in for a facility's NPDES permit except the sampling results, reporting period, and signature of responsible official.  A permittee then uses these preprinted forms to submit their monthly, quarterly, semi-annual, etc. sampling results.  In this way, a permittee only has to fill in the the sampling results and the reporting period in the appropriate blanks on the preprinted DMR.

Since only one copy of each type reporting period DMR is provided to a permittee by the Bureau, permittees must make sufficient copies of their preprinted DMR(s).  The copies are then used for reporting the sampling results during the term of a permit.  For example, a permittee with a five year permit and monthly monitoring requirements must submit 60 DMRs over the life of the permit.  Therefore, 60 copies of the preprinted DMR form must be made by the permittee for use in monthly submittal of self monitoring results.

The Bureau sends preprinted DMRs to permittees after a permit is issued and before the permit is effective. Typically, if an appeal is not filed on a permit, the preprinted DMRs are sent to the permittee about 20 days after the issue date. If an appeal is filed, the preprinted DMR is not sent until after the appeal is resolved.

While the Bureau initially provides only one preprinted DMR for each type reporting period, additional copies will be provided upon request when the original preprinted DMR becomes worn, lost, etc.; The Bureau contacts for preprinted DMRs are as follows:

Some permittees develop their own preprinted DMR forms or use DMR generation computer programs.  These DMR forms are acceptable to the Bureau provided they are first approved for use by the Bureau.  Therefore, for a permittee to use their own forms, a copy of their preprinted DMR must be submitted to the appropriate contact given above for review and approval.  Once approved, a permittee generated preprinted DMR form may be used.  If a permittee generated preprinted DMR form is used before it is approved for use, the Bureau may not accept the data submitted on the unapproved DMR form.
